(च) र्दि हााँ, तो ब्र्ौरा िें 4 लेखा परीक्षक का जववरण 4 (क) *लेखा परीक्षक की जनर्ुजि के जलए फाइल दकए गए प्रासंजगक प्ररूप के एसआरएन को जनर्ियष्ट करें (ख) *संबंजधत लेखा परीक्षक की जनर्ुजि की तारीख (दिन/माह/वषय) (ग) *लेखा परीक्षक की श्रेणी व्यजि ऑजडटर की फमय (घ) *लेखा परीक्षक की सिस्ट्र्ता संख्र्ा (ङ) फमय पंिीकरण संख्र्ा (च) वह अवजध जिसके जलए लेखा परीक्षक जनर्ुि दकर्ा गर्ा था * से (दिन/माह/वषय) *तक (दिन/माह/वषय) 5. *क्ट्र्ा लेखापरीक्षकों को हटाने के जलए कोई जविेष सूचना प्राप्त हुई है हााँ नहीं (क) र्दि हां, तो नोठटस प्राप्त होने की तारीख का उल्लेख करें (दिन/माह/वषय) [भाग II—खण् ड 3(i)] भारत का रािपत्र : असाधारण 9 (ख) ऐसी सूचना िेने वाले सिस्ट्र्ों द्वारा धाठरत पूंिी का प्रजतित र्ा िेर्रों/गारंटी द्वारा सीजमत कंपनी के मामले में सिस्ट्र्ों की संख्र्ा का प्रजतित 6 *क्ट्र्ा संबंजधत लेखा परीक्षकों को सभी िेर् लेखा परीक्षा िुल्क का भुगतान कर दिर्ा गर्ा है हााँ नहीं (क) र्दि नहीं, तो बकार्ा राजि का उल्लेख करें 7 ऐसे लेखा परीक्षकों द्वारा कंपनी को प्रिान की गई अन्र् सेवाओं का ब्र्ौरा 8 *लेखापरीक्षा का लंजबत होना अथायत उन जवत्तीर् वषों की संख्र्ा जिनके जलए लेखापरीक्षा लंजबत है 9 ऐसे प्रत्र्ेक जवत्तीर् वषय के जलए कंपनी के खातों की अवस्ट्था जिन्हें अभी बोडय द्वारा अनुमोदित दकर्ा िाना है र्ा िो बोडय द्वारा अनुमोिन हैं लेदकन अभी तक लेखा परीक्षकों को सौंपे नहीं गए हैं र्ा बोडय द्वारा अनुमोदित हैं, लेखा परीक्षकों को सौंप दिर्ा गर्ा है लेदकन लेखा परीक्षा अभी तक पूरी नहीं हुई है र्ा लेखा परीक्षा पूरी हो गई है, लेखा परीक्षकों द्वारा अभी तक मसौिा ठरपोटय नहीं िी गई है (क) (ख) (ग) जवत्तीर् वषय की समाजप्त जतजथ (दिन/माह/वषय) खातों की जस्ट्थजत जववरण 10 *क्ट्र्ा लेखा परीक्षकों के कब्िे में लेखा पुस्ट्तकों के संबंध में कोई जववाि है िो हााँ नहीं कंपनी को वापस नहीं दिए गए हैं संलग्नक (क) *लेखापरीक्षक को हटाने की मांग के आधारों का ब्र्ौरा (ख) * प्रजतवािी/प्रत्र्ुत्तर लेखापरीक्षक को सेवा का प्रमाण (ग) वैकजल्पक संलग्नक -
